What is apheresis?
Apheresis is a medical procedure that involves removing whole blood from a donor or patient and separating the blood into individual components so that one particular component can be removed. The remaining blood components then are re-introduced back into the bloodstream of the patient or donor. Apheresis is used for the collection of donor blood components (such a platelets or plasma) as well as for the treatment for certain medical conditions in which a part of the blood that contains disease-provoking elements is removed.
With regards to the collection of components, we at Sri Lanka do plateletpheresis to harvest platelets from healthy donors who have donated whole blood previously. This is done at several centres throughout the island. The benefit of apheretic platelets is that, the full adult therapeutic platelet dose or more can be harvested by a single donor.

In the same manner, plasma, white blood cells, Red blood cells and stem cells from peripheral blood can also be harvested from healthy donors. Out of these, Peripheral blood stem cell harvest, from autologous donors are done at Apeksha national Cancer Institute.

Apheresis procedure is done in a Therapeutic manner to treat the patients as well. This means we can remove the plasma of a patient who has bad antibodies and replace it with good plasma from healthy donors. Leptospirosis is one such disease of local significance that has shown favourable outcome with Therapeutc Plasma Exchange. There are many other conditions where this procedure is the first line therapy. In the same manner we can remove excess platelets in conditions where an excess of platelets are formed. We can remove red blood cells damaged due to various reasons and replace it with good healthy red blood cells.
These therapeutic procedures are done in Sri Lanka by the Consultant Transfusion Physicians Sri Lanka College of Transfusion Physicians and the doctors linked to the National Blood Transfusion Service, placed all throughout the country.

With the COVID 19 Pandemic, there are two instances where this Apheresis has come to the limelight.

1. Apheresis platelet donation
In order to cater for the need for the platelet, specifically in cancer patients, harvesting platelet alone from suitable donors has become more feasible, since the need is specifically for platelets. This is done at special centers including National Blood Centre Narahenpita, Apeksha Hospital Maharagama, Kandy Hospital, Kamburugamuwa blood centre and Jaffna

2. The convalescent plasma harvest
The U.S. Food and Drug Administration (FDA) has updated its rules around use of experimental treatments for the ongoing COVID-19 pandemic to include use of “convalescent plasma,” in cases where the patient’s life is seriously or immediately threatened.
Convalescent plasma is the plasma of patients who recovered successfully from COVID 19, rich in antibodies against COVID 19. It is an old method of passive immunity used in several others disease conditions.
A plasma exchange procedure is done to harvest the plasma of these patients who’ve recovered from COVID 19. This plasma, after checking for viral markers including COVID-19 can be used to treat critical COVID patients.

குருதிப் பிரித்தெடுப்பும், குருதி மாற்றீடும்

செங்குருதிக் கலங்கள், வெண்குருதிக் கலங்கள், குருதிச் சிறுதட்டுக்கள் மற்றும் குருதிப் பாய்மம் எனும் கூறுகளைக் குருதி கொண்டிருக்கின்றது. இக்கூறுகளில் வேண்டியவற்றை மட்டும் தனித்தனியே பிரித்தெடுத்துவிட்டு, அதற்கான மாற்றீட்டுடனோ அல்லது மாற்றீடு இல்லாமலோ எஞ்சிய கூறுகளை மீண்டும் உட்செலுத்துதல் Apheresis எனப்படும்.
இந்தச் செயன்முறை இரு வகைப்படுகிறது. முதலாவது, நோயாளிகளில் மேற்கொள்ளப்படும் Therapeutic Apheresis. நோய்வாய்ப்பட்ட ஒருவரின் குருதியில் நோய்க் காரணியைக் கொண்டிருக்கும் பாய்மம் அல்லது நோயால் பாதிக்கப்பட்ட கலங்கள் என உரியவற்றை மட்டும் பிரித்தெடுத்து, மாற்றீட்டுடன் (அல்லது இல்லாமல்) எஞ்சிய கூறுகள் நோயாளிக்கு மீளத் தரப்படும்.
உதாரணமாக Leptospirosis எனப்படும் எலிக் காய்ச்சலினால் பாதிக்கப்பட்டு தீவிர நிலையிலிருக்கும் நோயாளிகளுக்குக் குருதிப் பாய்மம் பிரித்தெடுக்கப்பட்டு ஆரோக்கியமான ஒருவரின் பாய்மம் மீளவும் உட்செலுத்தப்படுகின்றது.

இரண்டாவது வகை, ஆரோக்கியமான குருதிக் கொடையாளிகளில் மேற்கொள்ளப்படுகிறது. இரத்த தானம் போலவே தனித்தனியே குருதிக் கூறுகளைப் பிரித்தெடுத்து, தேவைப்படும் நோயாளிகளுக்கு வழங்க முடியும். இவற்றை இலங்கையில், குருதி மாற்று வைத்திய நிபுணர்கள், இரத்த வங்கி மற்றும் தேசிய இரத்த மத்தியஸ்தானத்தைச் சேர்ந்த வைத்தியர்கள் மேற்கொள்கிறார்கள்.

உலகளாவிய ரீதியில் Covid19 தொற்று ஏற்பட்டிருக்கும் இச்சந்தர்ப்பத்தில், இப்பொறிமுறை இரு காரணங்களுக்காகக் க‌வனத்தை ஈர்த்துள்ளது.

1.குருதிச் சிறு தட்டுக்களுக்கான தேவைப்பாடு: இவை வெறும் ஐந்து நாட்களில் காலாவதியாகி விடுபவை. தொற்றுநோய்ப் பரவலால் இரத்ததான முகாம்கள் இடைநிறுத்தப்பட்டு, இரத்ததானம் செய்வோரின் எண்ணிக்கை வீழ்ச்சியடைந்திருந்த போதிலும் குருதிச்சிறுதட்டுக்களுக்கான தேவை குறையவில்லை. புற்றுநோயாளிகள், இருதய சத்திரசிகிச்சைக்கு உள்ளானவர்கள், குருதி நோயாளர்கள் போன்றவர்களுக்குக் குருதிச்சிறுதட்டுக்கள் அதிகளவில் தேவைப்படுகிறது. இந்தக் காலகட்டத்தில் Plateletpheresis செயற்பாட்டின் மூலம் ஒரு குருதிக்கொடையாளரிடமிருந்து குருதிச்சிறுதட்டுக்களை மட்டும் அதிகளவில் பெற்று நோயாளர்களின் தேவையைப் பூர்த்தி செய்கின்றோம்.

2. Convalescent Plasma Harvest. : US-FDA (உணவு மட்டும் மருந்துக் கட்டுப்பாட்டுச் சங்கம்) இனால் பரிசோதனை சிகிச்சை முறையாக மேற்குறிப்பிட்ட செயன்முறை பரிந்துரைக்கப்பட்டுள்ளது. Covid19 தொற்றிலிருந்து முற்றிலும் குணமடைந்த ஒருவரின் குருதிப் பாய்மத்தைப் பிரித்தெடுத்துத் தீவிரமான நிலையிலிருக்கும் நோயாளி ஒருவருக்கு உட்செலுத்துதலே இச்சிகிச்சை முறையாகும். மீளவும் நோயளிக்குச் செலுத்த முன்னர், வைரஸ் தொடர்பான பரிசோதனைகள் மேற்கொள்ளப்பட்டு, இந்தப் பாய்மம் பாதுகாப்பானதென உறுதி செய்யப்படும். உடன்பாட்டு நோயெதிர்ப்புச் சக்தியை (Passive immunity) வழங்கும் இந்த சிகிச்சைமுறை வேறு பல நோய்நிலைமைகளுக்கும் பயன்படுகிறது. சீனாவில் இவ்வாறு தீவிர நிலையிலிருந்த நோயாளர்கள் சிகிச்சையளிக்கப்பட்டிருந்தனர். இலங்கையிலும் தற்போது பரிசோதனை முறையாக இந்தச் சிகிச்சை முறை வழங்கப்படவிருக்கிறது.
